Appointments
Office visits are by appointment only.
Appointments can be scheduled Monday – Thursday 9:00 – 4:30, Fridays 9:00am – 12:00 noon. To schedule an appointment or for all questions regarding the services we provide, please contact us by calling (615) 444-8686.
What to Bring to Your Appointment:
- Your insurance card
- A valid Photo ID
- Written referral from your doctor or authorization if required by your insurance company
- A list of current prescribed and over the counter medications
- Old and recent X-rays including CT scans.
- Co-payment and or deductibles.
Please note that all co-pays, uncovered charges and deductibles are due at the time of service.
Cancellations
When you must cancel or change appointment, please call 24hrs before your scheduled appointment.
