After completing the forms, please fax it to the office at (615) 444-7793 prior to your appointment.

If you prefer you can mail the completed forms to our office, but please ensure that you mail the completed forms several days before your appointment to allow enough time for mail delivery.

If you choose to fill the forms in our office, please arrive 30mins before your scheduled appointment time, to enable us enter your information into our database.
